The following items are required to service a case:

1. FOOD STAMPS / AFDC PROGRAM VERIFICATION
   • Referral letter 
   • Appointment verification form for food stamps/AFDC/TANF
   • Food stamp/TANF action letter 
2. RESIDENCE
   • Rent/Lease Receipt or Statement from Landlord 
   • Mortgage Payment Book/Statement 
3. COST OF SHELTER
   • Rent/Lease Receipt or Statement from Landlord 
   • Mortgage Payment Book/Statement 
4. INCOME (last 30 days)
   • Last four (4) pay stubs or strike verification 
   • Current unemployment card
   • Letter of amount of S.U.B. or letter of eligibility 
   • Social Security check or current letter of claim or ineligibility 
   • TANF/AFDC award letter or referral letter 
   • Written proof of child support (Court order) 
5. SIZE OF FAMILY
   • Social Security cards for everyone in household 
   • Birth certificates or hospital records for everyone in household 
   • Marriage certificate (if applicable) 
   • Divorce papers (if applicable) 
   • Green card if not a citizen of the United States 
   • Naturalization papers (if applicable) 
   • Driver’s License or Picture I.D. for anyone 18 years or older 
6. MEDICAL EXPENSES
   • Statement from doctor for special diets 
   • Statement from doctor for temporary disability 
   • Paid receipts for prescriptions 
   • Doctor’s receipts 
7. ASSETS
   • Savings account book 
   • Bonds, ERA’s, CD’s, 401-K’s, etc.
   • Checking accounts/books 
   • Credit Union Savings 
8. EXPENSES (last 30 days):
   • All Credit Card payments 
   • Utility Bills (including complete telephone bill) 
   • Car Loan(s) 
   • Cable Bill 
   • Other (i.e.; student loan payments, second mortgage payments, other Loans, etc…)
